Hospice House Social Worker Job Summary

• Responsible for provision of social work services to include crisis intervention, advance care planning conversations, resource referral and emotional support and/or counseling to patients and families as needed.
• Ability to work in coordination with other disciplines at part of team to address needs related to serious illnesses.

Hospice House Social Worker Job Requirements

• Masters Degree in Social Work with NC LCSW or LCSW Provisional license required. 
• At least 1 year of social work experience in any field. 
• Knowledge of end-of-life issues preferred. 
• Reliable transportation and valid NC Drivers license required.
